Insurance and Payment Information:
Matthew Sieradski, Matthew Branham, and Alicia Derby are preferred providers for most insurance plans, including PacificSource, Blue Cross/Blue Shield, Aetna, Lifewise, Providence, HealthNet, Cigna, ODS, OEA Choice, and Western Benefits. We also gladly bill automobile accident insurance. Alicia is credentialed with many worker’s compensation plans. For payment, we accept cash, check, VISA, MasterCard, Discover Card, and debit cards.
